Please click on the 'Order Form' link below to download the Company
Store Order Form.
The form will open in your Internet browser but it is a Word document that you can fill out
on your computer, save, and then email to
store@aerovent.com. Or, if you prefer, you can print the form, write
down your information, and then fax it to (763) 551-7501.
All logos embroidered on shirts will be placed on the upper
left chest. Logos on hats will be placed in the front center of the
hat. The logo size will be 3.5" wide on all apparel.
Stock items will be shipped within 24 hours and non-stock items will ship within two weeks. If you have any questions
